The Meet and Greet gives us the opportunity to get to know you and your pet(s), answer all questions, and determine what services best suit your needs. We will explain all our offerings, fees, and policies when we meet with you.

I hereby acknowledge this release from liability for accidental injury or illness that my pet may incur as a result of receiving services from StrutnPup. I agree to disclose any physical limitations, disabilities, ailments, behavioral issues, or impairments which may affect my pet's ability to receive services.

I authorize emergency medical care to be provided by the above named veterinarian/clinic or an appropriate alternative to be determined by StrutnPup, LLC in the event that the clinic/veterinarian named above is not available. I agree to reimburse StrutnPup LLC for any charges related to emergency care.

I agree to be responsible for all fees for services rendered, cancellation fees, and other fees as mutually agreed upon.
